Student Athlete Eligibility

Below you will find the information, forms, and resources required for student-athlete eligibility at Florida Christian School. All student-athletes must be cleared by the Athletic Department before participating in any tryouts, practices, or competitions.

EL2 + EL1 Sports Physical Form(s)

ECG screening and completion of the EL1 form are required for all incoming 9th-grade student-athletes, as well as any student-athletes in grades 10–12 who did not participate in athletics during the previous school year. While not required for all returning student-athletes, we strongly recommend that every student-athlete participate in this important screening.  Learn more from the FHSAA here

Jackson UHealth Urgent Care provides free ECG screenings for student-athletes. Parents simply accompany their student to any participating location and request a student-athlete ECG at check-in. Results, interpreted by a pediatric cardiologist, will be available through the Jackson patient portal within 3–4 business days. GA4 - Recruiting Policy Affidavit Form
